When you have a change of circumstance you must inform the NDIA by filling out the form so that your support, funding or even just your details can be adjusted accordingly. A change of circumstance can include your living arrangements, any financial changes, levels of support needs, or health conditions.

What is an NDIS change of circumstance form?
An NDIS change of circumstance form is a document used to notify the National Disability Insurance Agency (NDIA) of any changes in a participant’s circumstances that may affect their NDIS plan or funding. Letting the NDIA know of any current or future changes that might affect your plan or your eligibility for the NDIS is crucial.
This form is typically used to report changes such as changes in living arrangements, health conditions, support needs, or financial circumstances. It allows participants to update their information so that their NDIS plan can be adjusted accordingly to ensure they continue to receive the appropriate level of support and NDIS funding.
What is an example of a change of circumstances?
Some examples of life changes that you may experience that will require you to fill out an NDIS Change of Circumstances form include:
- Changes in living arrangements, such as moving to a new address or residence.
- Changes to your contact details, such as your phone number or email address.
- Changes in health conditions or disabilities, including new diagnoses, changes in severity, or improvements.
- Changes in support needs, such as requiring additional or different types of support services.
- Changes in financial circumstances, such as changes in income, or eligibility for benefits.
- Changes in employment status, including starting a new job, changing jobs, or transitioning to or from employment.
- Changes in relationships or caregiving arrangements, such as getting married, divorcing, or taking on new caregiving responsibilities.
- Changes in personal circumstances, such as change in legal status or guardianship arrangements.
- Changes in eligibility for NDIS funding, including changes in residency status, moving overseas or other eligibility criteria.
- Changes in goals or aspirations, such as changes in education or career paths that may impact support needs.
- Changes to your name, pronouns or gender.
- If you receive compensation for an accident or illness related to your disability.
- Any other significant changes that may affect the participant’s ability to access or benefit from NDIS supports and services.
How to submit a change of circumstance form in NDIS?
There are a few ways to inform the NDIA of changes in circumstances and you may find one way easier than another. You can call the NDIA on 1800 800 110 or email the information but using the form ensures you have provided all the relevant information that the NDIA will need to process your change request.

Make sure you fill out the form accurately and completely, providing details about the change in circumstances and include any relevant information or documentation to support your request.
